Student Conduct Incident Report Form

If you believe that you have witnessed or been a victim of a violation of the Student Conduct Code, utilize the incident report form.

Student Conduct Record Request Form

To request information regarding findings of responsibility within your student conduct record, please submit this form.

Expungement Request Form

If eligible, students can fill out the Request to Expunge Student Conduct Record Form. The OSCCS will contact the student once the decision is made. If the request to expunge student conduct records is granted, OSCCS will make a designation on the students file that the case has been expunged.
