Bias Report and Support System (BRSS) Form
The university developed a system through which students, faculty, staff and community members who have experienced or witnessed incidents of bias, prejudice or discrimination involving a student can report their experiences.
Student Conduct Record Request Form
To request information regarding findings of responsibility within your student conduct record, please submit this form.
Expungement Request Form
If eligible, students can fill out the Request to Expunge Student Conduct Record Form. The OSCCS will contact the student once the decision is made. If the request to expunge student conduct records is granted, OSCCS will make a designation on the students file that the case has been expunged.
